Annual Faculty Performance Appraisal (AFPA)

The Annual Faculty Performance Appraisal Form (AFPA) is a self-assessment tool that provides a summary of faculty achievements. The AFPA can serve as a check-point to determine whether faculty are keeping pace with expected productivity within their colleges/school and institution.

Process:

Step 1.  Pre-Form Completion

Step 2.  Annual Faculty Performance Appraisal (AFPA) Submission 

  • A link to the web-based AFPA form will be emailed from the Deans’ Offices to faculty members in perspective colleges/school. 
  • Faculty members will have three weeks to complete and submit their AFPA. 
  • The Web-based form will be available Friday, July 7, 2017 through Friday, July 28, 2017 and can be completed at different points of time (Note: Faculty must use the same computer and browser in order to continue saved work.).
  • A copy of the completed form will be generated for faculty records.  Please download your response summary (the completed form) print, and email a copy to the program director, department chair, and the PI/Division Chief.  


Step 3.
  Annual Faculty Appraisal Consultation

  • The formative nature of this activity requires a review and consultation with the Dean, Director, Chair, or Division Chief of the college/school.  Subsequent to review of the AFPA summary report, the Annual Faculty Performance Appraisal Consultation Form should be used to share feedback.
