Welcome to the Student Health Insurance webpage. This page is designed to provide students with general information about CDU’s Student Health Insurance Policy and links to details that students need to use the insurance plan.

CDU Policy
Based on CDU student input in December 2011 and effective fall 2013, CDU requires allfull-time undergraduate and graduate students to maintain adequate health insurance that covers them every day of their higher education career while enrolled at CDU.
CDU’s health insurance plan is underwritten by United Healthcare Insurance Company and serviced by Gallagher Koster.

Note: there is not a break in coverage between semesters unless a student is no longer enrolled (has withdrawn from the University, taken a leave of absence or graduated.)

CDU’s health insurance plan is underwritten by United Healthcare Insurance Company and serviced by Gallagher Koster. Gallagher Koster is one of the nation’s leading full-service, independent insurance program administrators for education communities. For each college/university they serve, Gallagher Koster creates a student-friendly webpage for students to access benefits information, preferred providers and helpful resources and links. The bolded link below connects you to the Gallagher Koster site.

Welcome Students and Scholars to Gallagher Koster/CDU

From the GallagherKoster/CDU website, students can:

  • Find a Doctor on the Preferred Provider List (A Preferred Provider is a doctor, hospital or other health care provider that participates in the UnitedHealthcare Choice Plus Network.)
  • Download a Plan Brochure or Frequently Asked Questions Document
  • Click on Account Home to view their personal coverage information
  • Edit their primary address
  • Generate an ID Card (feature available after students create an account with a unique username and password)
  • Engage in live chats using the "Live Help" icon to speak with a Gallagher Koster representative
  • Contact Customer Service to submit a change of address, request an ID card, submit feedback about Gallagher Koster, or ask miscellaneous customer service questions.

Preventive Services
The insurance policy includes a Preventive Services benefit for annual physicals, GYN exams, routine screenings and immunizations.

Enrolling or Waiving CDU’s PlanStudents who already have health insurance coverage do not have to enroll in CDU’s Plan; however, they must waive the plan at the start of their enrollment. A student that misses the waiver deadline during their 1st semester of enrollment can opt out of the plan during any subsequent semester by completing the Waiver during the Waiver Periods of each term (see Waiver Periods below). Detailed information about the process for waiving CDU’s Plan is emailed from Student Service to students at the start of every semester. Insurance information is always sent to students at their email accounts. Note: International students are enrolled on a mandatory basis and cannot waive coverage.

Enrollment in the Plan is set up as a Hard Waiver Program. This means that students who have comparable insurance can waive CDU’s plan by simply completing a Waiver during the Waiver Periods of each term (see Waiver Period below). Students who do not waive the Plan by each semester’s Waiver deadline are automatically enrolled in CDU’s health insurance plan. There are no forms to complete in order to enroll in CDU’s Plan; students simply skip the waiver and they are automatically enrolled in the Plan.

All full-time undergraduate and graduate students attending CDU must enroll in or waive the health insurance coverage offered by the University by the deadlines below.

Includes students in the Rad Tech, Life Sciences, Post-Baccalaureate and Urban Public Health programs.




Includes students in the MSN Program (ELM, FNP and ANE Tracks) and the Post-Master Certificate Program (PMC).




Students who wish to waive CDU’s Plan must confirm by deadline dates above that their existing coverage is at least equal to the Plan offered by the University. Students who do not provide the necessary proof to waive the insurance by the deadline will be automatically enrolled in the Plan offered by CDU.


Health Insurance Waiver Instructions – 5 Easy Steps

Note: The Waiver is an online process only and no paper forms will be accepted. If students determine their coverage to be comparable and would like to waive CDU’s student health insurance plan, they must follow the 5 easy steps below.

  • Go to
  • Click on ‘Student Waive’ link.
  • Create a user account, or log in if you are a returning user (Note: You will need the 5-digit number from your Student ID card for the User ID field, and please use all lowercase letters (no symbols or numbers) for all other fields.)
  • Select the appropriate waiver form:
    • ‘Waiver Form – School of Nursing’
    • ‘Waiver Form – College of Science and Health’
  • When waiving the insurance, have your current health insurance ID card ready as you will need this information in order to complete the waiver form. Immediately upon submitting your online form, you will receive a confirmation number. Please save this number and print a copy of the confirmation for your records. The confirmation number is for your records only and does not need to be provided to CDU to complete the process.

Note: International students are enrolled on a mandatory basis and cannot waive coverage.
