Description:
Under general direction of staff, provides health promotion services to patients and/or target population across a set of related project/program components. Responsible for providing outreach services to patients and/or target population with HIV/AIDS. Educates and connects clients with appropriate resources and support groups: Provide outreach services to individuals with HIV/AIDS and connect them with appropriate resources and support; acts as a liaison between community-based organizations serving the needs of clients and the EIP clinic; addresses risk factors and methods to reduce harm to clients and others. Establishes a plan of action and reviews with the Director; follow up with individuals who were once receiving medical treatment, but failed several medical appointments; document all activities conducted with clients; act as a client advocate in ensuring that all medical and non-medical needs of the client are being met through direct intervention and appropriate referrals; assists in client record data collection, input and retrieval as needed; educates and connect clients with appropriate resources and support groups; provides HIV/AIDS education and risk reduction interventions to at risk populations; meet with Program Director and Medical Provider once a week; attends community health fairs, social and recreational events; submits regular activity reports; assists with the development and implementation of program plans and activities; and adheres to policies and procedures for maintaining confidentiality.

Requirements:
High School diploma or equivalency required. Health Educator Certification in HIV/AIDS. 1-3 years of related experience providing outreach counseling and supporting health services. Knowledge of the impact of HIV/AIDS in ethnic minority communities; knowledge of HIV pre/posttest counselor policy and procedure guidelines; maintains effective working relationships with the public and staff; good communication skills; and works well with others. Bilingual (Spanish/English) proficiency in speaking and writing. Valid California class “C” drivers’ license.
