Required Documentation for Applying to the Program:
A current copy of your TB screening taken within the last 6 months (Options I & II applicants)
High School Diploma or GED
Equivalency required for all Options
Students who attended foreign institutions must have their transcripts evaluated by a State-approved transcript evaluation services provider. If you have attended college with 3 semesters or more can be used as secondary proof of higher education
Options III & VI must have documented proof of required number of draws from an employer within the last five years of employment. If you can not provide proof student must apply for Option II.
If you have been convicted of a felony or any misdemeanor other than a minor traffic violation, the state of California may deny your application. If you have any questions or concerns prior to enrolling please contact Pamela Ferrell by e-mail at pfarrell@dhs.ca.gov.
